Add / Drop / Withdraw form

If a schedule adjustment is being done that involves two different semesters, a form must be submitted for each semester separately.

Drop: Full refund and removed from academic record. Must be submitted by official semester drop date.
Withdraw: "W" recorded on academic record after official semester withdrawal date.
